Contrast material-enhanced myocardial perfusion imaging by using cardiac magnetic resonance (MR) imaging has, during the past decade, evolved into an accurate technique for diagnosing coronary artery disease, with excellent prognostic value. Cardiac perfusion studies with MR rely primarily on the use of T1-weighted techniques, while for brain perfusion it is more common to use T2*-weighted techniques. The reason lies in the difference in the distribution of the volume of gadolinium chelates, which normally cannot cross the blood-brain barrier, and the higher vascular volume in the heart compared with the brain.

There are three techniques in wide use to derive one or more perfusion values: techniques dynamic susceptibility contrast (DSC) MR perfusion; dynamic contrast enhanced (DCE) MR perfusion Dynamic contrast-enhanced (DCE) MR perfusion, sometimes also referred to as permeability MRI, is one of the main MRI perfusion techniques which calculates perfusion parameters by evaluating T1 shortening induced by a gadolinium-based contrast bolus passing through tissue. The most commonly calculated parameter is k-trans . Arterial spin labeling (ASL) MR perfusion is an MR perfusion technique which does not require intravenous administration of contrast (unlike DSC perfusion and DCE perfusion ). Instead it exploits the ability of MRI to magnetically label arterial blood below the imaging slab. The parameter most commonly derived is cerebral blood flow (CBF) . MR perfusion imaging refers to several recently developed techniques used to non-invasively measure cerebral perfusion via assessment of various hemodynamic measurements such as cerebral blood volume, cerebral blood flow, and mean transit time.
